To update details like your name, address, phone numbers or email address:
- use your NHS account (through the NHS website or NHS App)
- use your Patient Services account
- fill out a change your personal details form
Change of address
If you have moved to a new address, you will need to be in our catchment area to stay registered with our surgery.
You can only be registered at the practice, if you currently live full time within our defined boundary.
If your address is outside the boundary, then you will need to register at another practice nearer your new address.
If your new address is close to the edge or you think you should be allowed to register/re-register, then send us an enquiry.
Changing your phone number or email address
You will need to show us identification before we can change your phone number or email address.
Changing your name
You will need to show us proof that you have changed your name. This could be a marriage or civil partnership certificate, deed poll or birth certificate.
