Child new patient registration

Use this service to register a child with the surgery.

You can use this service if you:

• want to register a child under the age of 16 at the surgery
• are registering a child that lives within our catchment area

Before you start

We’ll ask you for:

• your first and last name, date of birth, sex, postcode, email and phone number
• if applicable, the details of the person you are completing the form on behalf of
• your NHS number
• information about your health
• other personal details and demographics
